How to make an insurance claim through Ontario Soccer (formerly the OSA):

Prior to making a claim please inform the office at the BSC so that we may assist you with what you require to.  Ontario Soccer (formerly the OSA) has provided the following information.

The Insurance section of Ontario Soccer’s website has all of the information and direction for these items which can be found here (follow this link).

 

There are two types of forms on the website:

​

1.  Request for Insurance Certificate

2.  Accident Claim forms (5 pages in total)

Ontario Soccer has requested we NOT download the PDF forms but suggests directing members to their website is the best way to ensure your forms are always up to date.
